ADHD Service Providers: Enrollment Guide

Enrolling in the Directory is a multi-step data entry process. Please review this guide before you continue.

This is only a guide! Do not try to click on the form fields below, because they are only sample screenshots of the real thing. After reviewing this guide, you can continue on to the actual process using the link at the bottom of the page.

Screen 1: Select a Membership Type

Select the second choice if you wish to become a member and list your services in the Directory (as shown in the graphic). Select the third choice if you wish to have only a listing in the Directory without becoming a member of our organization.

Screen 2: Enter Your Email Address

Enter a valid email address where you can receive confirmation emails and other business communications for your account. Be sure to enter it correctly, because it is your unique identifier and is used every time you login to your profile.

Screen 3: Profile Information

Directory Category

Select the one category that best describes your service or business. If you would like to be listed under multiple categories, you must create a separate profile for each. To create another profile, repeat the Member Enrollment process using a new email address. Contact your ISP if you need another email account for this purpose. Most ISPs will give you additional email addresses at no charge and allow you to have the mail forwarded to your primary mailbox.

Personal Information

This section is fairly self-explanatory. A few fields, however, can be confusing....

  • "Last Name" will often contain extra information. Be sure to include any suffixes (e.g., Jr, Sr, III) as well as any recognized abbreviations for professional degrees (e.g., M.A., M.S.W., M.Ed., M.D., Ph.D) and certifications (e.g., RN, LSW, LMFT, LMHC, ACSW, MCC) as you would list them after your name.
  • "Country" needs to be selected, even if you're in the U.S. It is the first item in the list.

Screen 4: Additional Profile Information

Screen 5: Additional Profile Information by Category

Complete the ONE section that corresponds to the directory category you selected (see Screen 3 above). In the example below, Jane Doe is a clinician/therapist, so she would complete only Section 4. If you enter data in more than one section, your profile will not display properly. If you would like to be listed under multiple categories, you must create a separate profile for each. To create another profile, repeat the Member Enrollment process using a new email address.

Screen 6: Summary

You are now ready to confirm your membership charge before proceeding to the payment information screen. There will be no charge if you chose Non-Member with listing in National ADHD Directory.

Screen 7: Payment Information

Your membership can be paid for using either a Visa or Mastercard. Press the Pay button when you are ready to submit this transaction.
