National ADHD Directory Participant Agreement
Each Directory participant must agree to the terms below. To give your consent, select Yes at the bottom of the page and click on the Continue button. You will then be able to create a new profile or edit your existing profile.
I am aware the National ADHD is intended, in part, as a listing of clinicians and therapists who have received at least a Master's Degree in a field related to ADHD and are in good standing with every governmental authority that regulates their practice as a listing of clinician or therapist. I am also aware that ADD Resources, the sponsor of the National ADHD Directory, supports the use of appropriate medicines for the treatment of AD/HD and does not want to list in its National ADHD Directory persons that are opposed to the use of medicines for the treatment of AD/HD.
By listing myself, or permitting myself to be listed in the National AD/HD Directory, I agree that I am representing and warranting to ADD Resources and to users of the National AD/HD Directory, that I have at least a Master's Degree and that I am in good standing with every governmental agency that regulates my practice as a clinician or tharapist. I also agree I will not speak, write, display on an internet website, publish, distribute literature, or otherwise communicate the opinion that medicines are not an acceptable treatment for AD/HD.
I agree that will list myself or permit myself to be listed, in the National AD/HD Directory, only the categories in which I actively provide services for which I am qualified.
I acknowlege and agree that my listing in the AD/HD Directory is a matter by grace by ADD Resources, that I do not have a right to be listed, and that ADD Resources may remove my listing(s) from the NAtional AD/HD Directory with or without notice and without cause providing a pro-rated refund, if any, is due.
