Duke MyChart is an electronic medical record patient portal that offers personalized and secure online access to portions of your medical records. The Duke MyChart secure website and app also makes it easy for you to schedule appointments, pay your bills, and send a message to your doctor’s office when you have non-urgent medical needs.

If you need help with your Activation Code please call Duke MyChart Customer Service at 919-620-4555 or 800-782-6945 between 8:00am-5:00pm ET Monday, Tuesday, Wednesday and Friday or 8:00am-4:00pm ET Thursday. If you are a new patient to Duke and wish to request a new patient appointment complete the New Patient Appointment Request form.
