Creating a MyCHART account is quick and easy. Start by logging on to mychart.erlanger.org and begin the registration process. When you log into My Chart for the first time, you will be prompted to create your own unique My Chart user name and password. Access My Chart website to start the registration. Log on to mychart.erlanger.org to begin

A MyChart account will be activated for both the proxy and the account holder. If the proxy’s legal relationship with the account holder changes, the proxy must inform the patients Erlanger clinic immediately by phone, or sending written notice to Erlanger Health System, Attention MyChart, 1501 Riverside Dr., Suite 300, Chattanooga, TN 37406.
