Georgia Families gives you choices for your health care! Choose a health plan and a Primary Care Provider for each member in your family who is in Medicaid or PeachCare for Kids ® . As soon as you become a member of Medicaid or PeachCare for Kids ® you will be enrolled into a health plan.

Georgia Families® is a program that delivers health care services to members of Medicaid and PeachCare for Kids®. The program is a partnership between the Department of Community Health (DCH) and private care management organizations (CMOs).

Georgia Families 360° Managed Care for Foster Care, Adoption Assistance and Juvenile Justice Members. Georgia Families 360°SM, is the Georgia’s managed care program for approximately 27,000 children, youth, and young adults in foster care, children and youth receiving adoption assistance, and select youth involved in the juvenile justice system.

Amerigroup Community Care is Georgia Families 360°SM's single care management organization (CMO) in charge of coordinating care for children in Foster Care, children receiving Adoption Assistance and select youth in the Juvenile Justice system.
